NICU Nurse Case Manager

Job Description:

A company is looking for a Nurse Case Manager - NICU who will focus on empowering families and advocating for necessary medical services.

Key Responsibilities

Assess members' needs and develop individualized care plans
Monitor care plans and coordinate services with healthcare providers
Provide education and referrals to community resources for members

Required Qualifications

Registered Nurse (RN) with a current, unrestricted license
Three or more years of clinical experience in a Level III or above NICU
Experience in a Case Management role preferred
CCM certification required within the specified timeframe
Experience with data entry into a database or EHR required
